The Practice covers patients living in Birches Head, Burslem, Cobridge, Etruria, Festival Heights, Festival Park, Ford Green, Forrest Park, Hanley, Longport, Middleport, Milton, Norton, Smallthorne and Sneyd Green.

Remember to inform the Practice should you change address, following which a decision will be made on whether you will need to register with another Practice locally or able to remain with this Practice.

Registration

This Practice always welcomes new patients to join.

We have an open list and are able to accept requests for registration from patients living or moving to a wide area from our Practice.

How do I register as a patient?

A registration application form is available on request at reception. Alternatively, you can download a PDF version of the patient registration form here.

Kindly complete a form for each person wishing to register with the practice and return this to reception. If you have recently arrived to this Country from abroad, please bring either your Passport or Visa as proof of identification.

This form involves your past and present medical history, previous GP details, immunisation details for all children under the age of 6 years (can be obtained from your child’s Red Book), and most importantly your NHS number (which can be obtained from your previous GP if not known by yourself). Reception staff will inform you when to return to the surgery to see if you have been accepted by the Practice.

Following acceptance, new patients aged 15 years and over will be given an appointment for a New Patient Health Check with our Practice Nurse.

Online 'Pre-Registration' With The Practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
